Arrears Follow a Process, Not a Cliff Edge
Falling behind on private health premiums is stressful, but it does not trigger sudden cancellation the way missing a gym payment might. Because health cover is compulsory, the law deliberately makes it hard to simply lose it — instead there is a defined sequence of reminders and, ultimately, a stripped-back emergency tariff that keeps you insured while the debt is dealt with. Knowing the steps turns panic into a plan.
You are not thrown out: for a compulsory Volltarif, unpaid premiums lead not to termination but, after due warnings, to a reduction into an emergency tariff. The cover narrows, but it does not vanish — the system is built to keep everyone insured.
The Stages of Falling Behind
- Missed payment and reminders: the insurer issues reminders (Mahnungen) with deadlines; interest and costs can accrue on the arrears
- Formal warning of consequences: the insurer must warn you, in the required form, that continued non-payment will move you to the emergency tariff
- Notlagentarif (emergency tariff): if the debt persists past the warnings, you are placed in the Notlagentarif — a minimal tariff covering acute and emergency treatment, pregnancy and certain other essentials, at a low premium while you clear the arrears
Getting Back to Full Cover
The Notlagentarif is a holding position, not a destination. Once you have repaid the outstanding premiums (including any interest and costs), you move back into your previous tariff automatically — with your ageing reserves preserved, which is the crucial point: you do not start again or re-answer health questions. That is why clearing the debt, even gradually by arrangement, matters so much: it is the door back to your full cover on your original terms.
If the Premium Itself Is the Problem
If arrears stem from a premium that has simply become unaffordable, address the cause in parallel: a §204 Tarifwechsel to a cheaper tariff within your insurer, a higher deductible, or — where eligible — the Standard or Basistarif, whose premiums are capped. Speak to the insurer early; payment arrangements and tariff changes are far easier to agree before the emergency tariff than after months of silence.
The Bottom Line
Unpaid PKV premiums lead to a staged process — reminders, a formal warning, then the low-cost Notlagentarif — rather than losing cover outright, and repaying the arrears restores your full tariff with reserves intact. If the premium is the real issue, fix it with a tariff change or safety-net tariff at the same time. Engage the insurer early: the whole system is designed to keep you insured, and it works far better when you use it.
